Partial hospitalization programs (PHP) in Orange County

A partial hospitalization program (PHP), called high-intensity outpatient in ASAM's current criteria, provides at least 20 hours of addiction treatment a week without an overnight stay. It can fit someone who is medically stable but needs more structure than IOP. In Orange County, verify the clinical program separately from any housing it recommends.

Continuum of addiction treatment intensity
  1. Hospital inpatient 24-hour medical care
  2. Residential / detox Live-in treatment
  3. PHP / high-intensity OP 20+ hrs / week
  4. IOP 9–19 hrs / week
  5. Outpatient Under 9 hrs / week
  6. Long-term monitoring Recovery checkups

Sober living is housing, not a level of clinical care — but ASAM can recommend a recovery residence alongside outpatient treatment.

What happens in a PHP

A PHP schedule commonly fills much of the weekday. Services may include:

  • Group and individual therapy
  • Medical and psychiatric assessment
  • Medication management when clinically appropriate
  • Family education or sessions
  • Drug and alcohol testing
  • Discharge planning for IOP, outpatient care, and recovery support

Exact schedules and services vary. Ask for a written weekly calendar and the credentials of the people delivering each service.

Who PHP may fit

PHP may be appropriate when a clinical assessment finds that you:

  • Are medically stable and do not need 24-hour nursing or withdrawal management
  • Need 20 or more hours of structured care each week
  • Can remain safe outside program hours
  • Have stable transportation and a drug- and alcohol-free place to sleep

PHP is not medical detox. Alcohol, benzodiazepine, or other potentially dangerous withdrawal needs prompt medical evaluation. If symptoms are severe or life-threatening, call 911 or go to an emergency department.

PHP, IOP, and residential care

Level Weekly structure Where you sleep
Residential 24-hour live-in setting At the treatment facility
PHP / high-intensity outpatient 20+ treatment hours Home or sober living
IOP 9–19 treatment hours Home or sober living
Standard outpatient Under 9 treatment hours Home or sober living

A clinician should recommend the level based on withdrawal risk, physical and mental health, relapse risk, living situation, and personal needs—not simply on which service a program sells.

DHCS does not publish PHP as a separate category in its facility dataset. It lists 120 Orange County outpatient programs, but a listing does not prove that a location offers ASAM Level 2.5 care. Call the program and ask whether it provides at least 20 hours a week, then verify the facility on the DHCS list. See how to check a California rehab license.

Questions to ask before choosing

  1. Is this ASAM Level 2.5 care, and how many clinical hours are provided each week?
  2. Who performs the assessment, and why is PHP the recommended level?
  3. How do you handle mental health conditions and prescribed medications?
  4. What happens if withdrawal symptoms or another medical problem appears?
  5. Which services are in person, and which are telehealth?
  6. What is the plan for stepping down to IOP or outpatient care?
  7. What are the total charges after insurance?

Pressure to enroll before an assessment, a promise of free rent or travel tied to attendance, or a refusal to identify the actual provider are red flags. See patient brokering and rehab scams.

Where you sleep during PHP matters

PHP accounts for part of the day; evenings, nights, and weekends happen elsewhere. A sober living home can provide a drug- and alcohol-free place, house rules, and peers in recovery between treatment days.

The District is sober living, not a PHP or treatment center. We have men's and women's houses in four Orange County areas. Couples live in separate District houses and can see each other at PHP, OP, and aftercare programs. We can connect residents with licensed programs near our homes; residents choose, and we do not pay or accept anything of value for referrals.

Common questions

What is PHP for addiction treatment?

PHP is high-intensity outpatient treatment for people who need most of the weekday in structured care but do not need to sleep at a treatment facility. ASAM Level 2.5 generally means 20 or more treatment hours a week.

Do you live at a PHP?

Usually not. You attend treatment during the day and sleep at home or in sober living. Confirm whether a program uses PHP to mean a true outpatient schedule or a residential service with different billing.

What is the difference between PHP and IOP?

PHP is more intensive, generally 20 or more hours a week. Adult IOP is generally 9 to 19 hours a week and is more compatible with work or school.

How long does PHP last?

There is no universal length. A clinician and insurer review progress and medical necessity, then plan a step down to IOP or another appropriate level.

Can couples see each other while attending PHP?

Couples in separate District houses can see each other at their PHP, OP, and aftercare programs, subject to the treatment program's attendance and contact rules.

Is The District a PHP provider?

No. The District is sober living, not treatment. We can help people compare licensed programs near our Orange County homes, and the person chooses the program.
